| The Short-term Care Division of Community Homecare provides home
health care services under a plan of care ordered by your doctor.
These services are of a sub-acute nature following a hospital stay, or
ordered by your doctor because of a diagnosis your doctor feels would
benefit from specialized care in your home. The home health services provided by our Short-term Care Division are skilled services by medical professionals in our organization. They usually have a short duration and may involve more than one specialty to help you recuperate as quickly as possible. KEY SERVICES:

The Long-term Care Division of Community Homecare provides home
health care services and assistance with activities of daily living.
