Personal Care Services

Our Personal Care program is designed to help elders and people with disabilities with Activities of Daily Living (ADLs) so that they may stay in their homes rather than have to go into a nursing home.

Eligibility

The following individuals are eligible for personal services:

Consumers who have full Medicaid coverage, generally through Supplemental Security Income (SSI).
Are twenty-one (21) years of age or older.
Meet nursing facility level of care.

Service Models

Consumers can choose either the Consumer-Directed or the Consumer-Delegated model. In the Consumer-directed model, the consumer is in charge of the delivery of services. She or he advertises if necessary, interviews and select her/his own attendants, supervises the attendant in providing the services and fires the attendant if necessary. In the Consumer-Delegated model, the consumer delegates these tasks to the our agency. Under both models, the attendant may be a family member.

Role of the Attendant

The role of the personal care attendant is to help the consumer with the Activities of Daily Living listed on her or his Individual Plan of Care (IPoC). Only certain services can be reimbursed by Medicaid so it’s important for attendants to be familiar with the covered services.

Covered Services

Consumers will be assessed both individually and jointly if sharing a living space with another applicant/recipient, in each of the following listed service categories. Our service will not include those services for tasks the individual does not need or is already receiving from other sources including tasks provided by natural supports.

Meal Preparation and Assistance:
1. Prepare meals including cutting ingredients to be cooked.
2. Cooking of meals and placing/presenting meal in front of consumer to eat.
3. Cutting up food into bite-size portions for the consumer.

Hygiene/Grooming – Performing or cueing and prompting with:
1. Bathing
2. Dressing
3. Grooming
4. Oral Care
5. Nail Care

Eating – Feeding the consumer or assisting the consumer with eating a prepared meal with utensils.
Household Services – To maintain a clean and safe environment for the consumer.
Supportive Mobility Assistance – Physical assistance or verbal prompting and cueing for assistance with:

1. Ambulation – Moving around inside or outside the consumers’ living area with or without assistive devices such as walkers, canes, and wheelchairs.
2. Transferring – Moving to/from one location/position to another with or without assistive devices including in and out of vehicles.
3. Repositioning – Turning or moving an individual to another position who is bed bound to prevent skin breakdown.
